PERSONAL DATA HELD BY SWFV

The data held by SWFV on our Practice Management System is provided by our clients, with some information such as a CPH number being available to us from APHA

  • Name

  • Address

  • Telephone number

  • Email address

  • Detailed instruction of how to get to your ground

  • Occasional note of family detail, for example, a death, where relevant to the running of an enterprise

  • CPH number

REASON FOR HAVING DATA

  1. To be able to speak to and/or visit farmers about their stock health and welfare

  2. May need multiple numbers/emails if more than one person is working on a farm

  3. To be able to bill clients

PROCESSING PERSONAL DATA

We process data with third parties – laboratories, APHA, insurance companies.

APHA and the Veterinary Investigation Centres are government run organizations

All other laboratories, we use the client number so rendering the report anonymous.

We do not share information with insurance companies unless specifically asked to by our client.

LENGTH OF TIME DATA IS KEPT

Data is kept for as long as a client is active, and for 7 years afterwards.

DATA SUBJECTS RIGHTS

  1. Organizations must tell individuals what data is being collected – all the information above.

  2. Right of access. An individual is entitled to request a copy of the information held by an organization about them

  3. Right of rectification. An individual has the right to correct any incorrect information held

  4. Right to be forgotten. An individual may request an organization to delete any information held about them

  5. Right to portability. An individual has the right to request the personal data held to be transferred to another organization

  6. Right to restrict processing. In some circumstances, an individual can request that an organization limits its use of personal date

  7. Right to object. Individuals have the right to object to some kinds of processing, such as direct marketing

  8. Rights related to automatic decision making, including profiling. In most circumstances, individuals have the right to object to activities where automatic decisions are made based on their personal data
